Is that the sort of thing you're after, or two small separate racks that bolt to the fork legs?
Google porteur racks for options, though they don't seem to use the 'bottle' bosses on forks.

bit overkill that Andy.
was looking for something with a single mount at the crown. the fork has a drilled threaded hole where a mudguard can mount.
so a single mount at the crown and a mount on each leg lower down.
some of those porteur rack would do. but how much as the price shot up since rack became a fashionable hipster item..............

Ah, right, thought you were after something bolting to the outer bottle mounts. I've a mate may be able to make you something if you can't find what you're after.

Not sure I can think of anything that bolts to the crown and the lowrider mount. The lowrider mount on the side is designed to be a specific distance from an eyelet at the dropout so distance between that mount and the crown will vary with fork length making it tricky to get a universal fit.
A lot of Rando/Porteur racks will bolt to bosses quite high up the fork leg, these won't go low enough for the lowrider mount and the orientation of the anythingcage mounts will make it difficult to fit even if they end up being the right height.
I'd assume that the hole through the crown is designed for mudguard/light?

This might be a bit overkill too but the Specialized Pizza Rack mounts at the crown and then at the fork legs. You can sling front panniers on it or use the top for a bag or a basket.
